Student Risk Screening Scale
The Student Risk Screening Scale (SRISS) is designed to identify students with behavioral or emotional patterns that impact academic achievement or social relationships. When completing the SRISS keep in mind that students behave differently for different people and in different settings.
The Student Risk Screening Scale can be downloaded below. Each advisory teacher needs to complete the SRSS class form three times per year (October, January and May.) The SRSS once completed is to be saved in the teams SIT Folder on the shared drive. After downloading the file save as last name, month /year. Example: (Lapka_Oct.17)

STUDENT IMPROVEMENT TEAM (SIT) PROCESS
(SIT) approach is a comprehensive systemic process that connects students, in all grade levels, with developmentally and educationally appropriate services to help them achieve their greatest academic, social, and personal well-being. The Student Improvement Team process brings together different systems, organizations and resources to maximize youth academic performance and resiliency.
The (SIT) process is designed to assist all students. It does not limit services to students who have been identified as "at-risk." While students who are referred may have shared or demonstrated certain risk indicators, the primary purpose for the referral is the observed behavior – not the fact that a student may or may not be considered "at-risk”. Before meeting
Accommodations- change how the child gets the information; do not effect the student’s grade
Modifications- changes are made to the content and performance expectations, does effect the student’s grade
- Identify kid and specific issues (cognitive, attitude, behavior, effort) – student needs to be a concern in a majority of classes, not just one or two
- Contact parent- must be a verbal contact, teachers with issues should call. Let them know they will receive a copy of the SIT plan teachers are going to be using to help the student be more successful.
- Read info on File maker (previous notes, test scores etc) and finish filling in necessary info.
- What strategies have been tried and what were the results
- Create a plan for the student
- Select and prioritize three strategies that will be implements
- How will the strategies be documented?
- Document use (when and where) of strategy
- Document the results of using the strategy
- Review progress on “kid day” in team
